We prefer that you download forms from our website, complete them electronically, and then print your forms to submit by fax to 903-526-2679. You may also bring the completed forms with you to your appointment. This site does not support the transmission of forms to the clinic. Forms are available for download as PDF files. You will need Adobe Acrobat Reader to view the form. Adobe Acrobat Reader is a free software program that allows you to view Portable Document Files (PDF). You may download it at no cost from Adobe's Web site.
